Wooden weight loss plaque personalised with your name. This handmade item comes in a wide variety of colours and is provided with chalk in its holder for ease of recording your weight loss in the chalk hearts. The hand painted board is created in white, but you may select a border colour of your choice which will be co-ordinated with the ribbon.

Hearts help you track your 'total lost', 'this week' and 'to lose' as you progress on your weight loss journey. The plaque can also be personalised 'Name's Weight Loss Tracker' and if you require this option, please do ensure that you complete the 'name' box when ordering. Other items are also available to compliment the plaque such as the diet jars and the record keeper plaque. Due to the personalised nature of this product it can not be returned.
